* condvar.c (ptw32_cond_wait_cleanup): Ensure that all
waking threads check if they are the last, and notify
the broadcaster if so - even if an error occurs in the
waiter.
* semaphore.c (_decrease_semaphore): Should be
a call to ptw32_decrease_semaphore.
(_increase_semaphore): Should be a call to
ptw32_increase_semaphore.
* misc.c (ptw32_cancelable_wait): Renamed from
CancelableWait.
(cond_wait_cleanup*): Rename to ptw32_cond_wait_cleanup*.
(ptw32_cond_timedwait): Add comments.
