* barrier.c: Remove static initialisation - irrelevent
for this object.
* pthread.h (PTHREAD_BARRIER_INITIALIZER): Removed.
* rwlock.c (pthread_rwlock_wrlock): This routine is
not a cancelation point - disable deferred
cancelation around call to pthread_cond_wait().
