* exit.c (pthread_exit): Add comment explaining the longjmp().

(_PTHREAD_NO): Define.
(RND_SIZEOF): Remove.
* create.c (pthread_create): Rename cancelability to cancelstate.
* pthread.h (pthread_attr_t): Rename cancelability to cancelstate.
(PTHREAD_CANCELED): Define.
